Health Record is a reference for the people responsible for releasing health information correctly, and for the patients and families making requests. This page explains how we source, write, and correct our content.
What we rely on
We link every regulatory claim to its authoritative source: HHS.gov, the HHS Office for Civil Rights, the eCFR (for the underlying 45 CFR text), HealthIT.gov, and CMS.gov, so you can verify it and cite it yourself. We do not summarize a rule without pointing to the primary text it comes from.
How we write our guides
We deliberately write from the requester's point of view, because that is the view a regulator will take when a request is answered too slowly, priced too high, or refused on grounds that don't hold up. Our guides describe process and rights rather than rating or recommending specific providers or vendors.
Corrections
Found an error — a broken source link, an outdated regulatory reference, or a factual mistake? Tell us and we correct it. Use the contact page to reach us.
Update cadence
Articles are reviewed periodically and updated when a cited rule or agency guidance changes. Each article notes when it was last updated.
